Please download and fill out these forms completely before coming in for your first appointment. They help provide very useful information to our therapists to improve your recovery and outcomes. We must have all forms filled out and signed prior to our therapists begining the first session.

What To Bring to your Orthotic Appointment

Ben will be your physical therapist for a dedicated one on one session to evaluate your feet, ankles, knees, hips, posture, and gait. Your outcomes are very important to our team. For that reason they ask that you arrive prepared and early. Here are just a few of the items that our staff would like you to bring. 

  • Paperwork Filled Out

  • Any Reports: Physician, X-ray, MRI etc..

  • Insurance Card and Identification

  • Credit Card or HSA Card

  • If in a current workout routine: Bring your training/running logs

  • Ideally what your goals are

  • Previous orthotics if applicable

  • Shorts

  • Running or Walking Shoes (last 3 pair)

  • Shoes you want orthotics to fit into


Cost of Orthotics

 Cost: $400.00+ tax (includes rush production, shipping, and handling) This is for the device only and the clinic visit is not included in this price. The office visit is typically covered by all insurance companies but may be subject to co-insurance, co-pay, or deductible payment. We will submit the orthotic charges on your behalf and refund you the difference if any payment is made. There are only a select few **employee medical insurance plans that cover orthotics consistently.

*Rush delivery typically runs 7-10 business days. In light of supply chain and lab labor shortages times may be extended.

**Insurances that have reliable coverage include: Microsoft, Boeing, Amazon, & Google and only require a payment toward deductible and DME coverage.

The orthotic price covers the cost of the orthotic device only. Patients and customers are still responsible for the office visits which runs approximately $150.00 (insurance will be billed for the evaluation if possible, per applicable contracts). With these combined costs we remain one of the most cost effective custom orthotic companies you can find in the Northwest.

What are Orthotics?

Orthotics

In layman's terms, you may think of them simply as arch supports or shoe inserts. However, they are actually much more than that. Orthotics are orthopedic devices made to: treat or adjust abnormal foot motion; provide support, stability, and control. They will increase an individual's balance, help to absorb shock, and relieve pressure on uncomfortable or sore spots. It is important to remember that your feet are the foundation of your body. The orthotic is rarely the singular fix and most commonly core and hip stability are a large part of the healing equation. 


Are Orthotics the same as an insole?

Most store bought inserts (also called insoles) are fabricated from nothing more than different density cellular sponge and/or foam rubber materials. They may provide some shock absorption, but little (if any) biomechanical control or support. If you can bend an insert in half, it is nothing more than an insole. Lake Washington Physical Therapy with Prolab manufactures true custom-orthotics, not rubber insoles. As an added convenience we do carry Superfeet on site if you are interested in trying an off the shelf option.


How are Orthotics made?

Orthotics are casted digitally in a non-weight bearing position. The digital casts are sent to our lab in Nappa California where they begin preparation and production almost immediately. Orthotics are currently created in about 2 weeks.


How Long Does it Take to Get my Orthotics

Approximately 7-10 business days during most years with the included “rush”. Orthotics casts are scanned via our state of the art digital scanner and emailed directly to the lab. This cuts down wait time by about 14 days. Ben and Katie will give you an approximate time for pick-up at which time you will have as scheduled therapy visit to be fit and a preliminary test of the orthotic will begin. Allow for more time near holidays as our lab observes all federal holidays.
